How to Obtain Vital Records from Italy

I hope this is useful for people looking to get Birth Certificates or other Vital Records from Italy. I just made this model letter with instructions as complete as I could. Post on the message board any questions you may have, or suggestions in how to improve it.
First of all you will need to buy one (or two) US$ 1.75 International reply coupon (IRC) at any USPS store (these are from the UPU or Universal postal union/Unione postalle Universale) then complete the fields with your info and your ancestor's, print this letter and post it to the comune with a "return receipt" and don't forget to include one or two IRCs.
Sample Letters can be found below the instructions.
*The letter should contain the fields which are in bold only. The words in italics refer to the information needed for filling in the blanks.*
This is the model letter to request information on birth, marriage or death if you know the date of birth of your ancestors:
Letter #1
AL SIGNOR SINDACO DEL COMUNE DI (place here the name of the comune where your Italian ancestor was born)
C.A.P. ( ) (zip code of that comune, you could find it at http://www.comuni.it/link/index.htm where after typing the comune's name and then "cerca" all the info is available)
PROV. DI (name of the province) - ITALIA
Il/la sottoscritto/a: (Pick "Il" if the requesting party -YOU, most likely- is a male, pick "La" if is a female, then write the requesting party's Last name, first and second name)
discendente del Sig. (italian ancestor's Last name, first and second name, if the ancestor is a woman then use "della Signora" instead of "del Sig.")
nato a (name of the comune where the ancestor was born)
il (date of birth written as dd-mm-yyyy - )
figlio di (Italian ancestor father's first name and second name)
e figlio di (Italian ancestor mother's last name, first name and second name ) <Note that if your ancestor is a woman you need to change "figlio" for "figlia" >
chiede cortesemente il rilascio di : (select with "X" all that applies)

( ) ATTO DI MATRIMONIO CON (this means Marriage Certificate. Only if you choose this option then write after "CON": spouse's last name, first and second name)

Letter #2
If you do not know when your ancestor was born or who the parents were you should send the following letter:

del mio nonno/a: if you are requesting your grandfather's BC choose NONNO, if your grandmother's choose NONNA, if it is your great grandfather's BC replace NONNO with BISNONNO and if it is your great grandmother then replace with BISNONNA and then write: Ancestor's Last name, first name and second name,
nato a (write here the Comune of birth)
(now choose one of the 2 options BUT only one if you pick option 1 do not copy the black part from option 2 and viceversa).
Option 1 in case you know when your ancestor was born, in this case write the date of Birth -il dd-mm-yyyy
Option 2: If you do not know the date of birth but a year range of not more than 10 years, you write
-circa YYYY (was born after...) - YYYY (was born before...)
Inoltre le richiedo gentilmente inviare il atto in carta semplice e con le eventuali annotazioni marginali.
